Forms:
For your convenience, we have provided 3 forms to assist you in
your various needs. Please download the form, fill it out and mail
it directly to Bill Mullen at
bill_stluke@verizon.net.
It would be prudent to save the file under your groups name and date its
submitted.
(For example Sunday School can make a Church request using
"checkrequest.doc" and save it as "SSCheck3-5-09.doc")
This will also be helpful for your record keeping as well as the
Church's since each of us will have a copy on file.
These documents are in the older Word version to avoid compatibility
issues.
